Last year, CareIQ helped automate the recall and review of patients with Hypertension, Diabetes and AF in a PCN covering approximately 90,000 patients and 14 practices.

We were able to do this through our Radar product that was developed by GPs working in the NHS.

Radar draws data from the EMR, identifies patients based on defined rules, risk stratifies them into high or low risk categories and allows communication and automation of administrative work such as recall of patients.

Our results illustrated increased productivity of healthcare staff. The online platform enabled staff to optimally manage their patients with chronic conditions. Patients were able to monitor their health and interact with their healthcare team. We were able to clearly demonstrate significant health improvements for patients and significant cost savings for the NHS.
